2. Clinical Evidence Hierarchy

We classify all recommendations using standard international evidence hierarchies:

Level A: Supported by multiple randomized controlled clinical trials or meta-analyses (e.g. PREVENT trial for subclinical bioimpedance surveillance; PAL trial for progressive resistance exercise).
Level B: Supported by single randomized trials or prospective registry cohorts (e.g. pneumatic compression pump trials, LYMPHA trial for immediate lymphatic reconstruction).
Level C: Supported by expert consensus, physiological principles, and institutional CDT protocols.
